Indirect Contact hour opportunities

Use this format for any contact with a client that is not a session; i.e. no show, late cancel, text/call between sessions, etc. This link will send you to Google Docs where you can save a copy to edit (File>Make a Copy). Consent to record session

When you're ready to record a session with a client, email me which client and I will send them this form via DocuSign for you and the client to sign.

Diagnositc Letter

For providing diagnosis to our clients and providers that may request them for alternate services or whatever it may be. Created by Leigha Myers.

Consent to observe session

Clients will sign this form so that you can observe counseling sessions. You will be prompted by DocuSign via email to sign as the observer.

No Harm Contract

If you think your client should sign a No Harm Contract, text me right away with the patient's name so that I can email them this No Harm Contract to sign via DocuSign. You may want to print a few copies to keep on hand for in-person clients to sign during your session (just upload it to Google Docs after signing and save with this format: client name_noharmcontract).
